  1. Sports Physical (turned into the nurse or coach before the first athletic contest)
  2. Completed Athletic Handbook Packet (turned into the coach before the first athletic contest)
  3. Participation Fee (either $50 or $35 depending on the sport) due in full by the date of the first game/contest. Failure to do so will result in the student's termination of participation. 
  4. Maintain Academic Eligibility

Bus Information

 All athletes have are able to utilize the activity bus, and the activity bus leaves West Middle School at 5:45pm Monday through Friday. All athletic events that are local to the Rockford area have a one-way bus to the event. All athletes are required to have arrangements made to have transportation home after all athletic events are over.
